What is CIITIZEN?
Ciitizen is a secure digital platform for storing all of yourexisting medical records, controlled by you, at no charge. If youchoose to share de-identified and summarized medical historywith researchers through your account, the information fromyour records will help us gain a better understanding of DLG4and get us closer to disease-modifying treatments.

What Do You Need To Know?
- Signing up is free.
- Ciitizen is currently for US patients only.
- It only takes about 15 minutes to sign up.
What Do You Need?
Before you sit down to sign up, gather the following:
- Computer or mobile phone with a camera
- Your Photo ID – Drivers License or Passport
- Your loved one’s birth certificate
- The hospital where your loved one received the DLG4 diagnosis and/or receives care
